Abstract

Abstract An improved trocar sleeve (100) is disclosed for use in laparoscopic surgery. The sleeve (100) is provided with an expanded mushroom hinge (112) at the first end (106) of the sleeve inserted into the abdominal cavity. The mushroom hinge (112) can be expanded with the abdominal cavity to abut against the inner surface of the abdominal wall (18) to hold the sleeve in place. A split seal (52) having a convex shape facing the first end (106) of the trocar sleeve (100) is provided to prevent loss of pressurized gas from within the abdominal cavity through the sleeve (100) as the pressure of the gas itself urges the first and second seal lips (78,80) on opposite sides of the slit in the seal into engagement. In a second embodiment, a member (138) is rotated relative to an end member (150) to actuate the mushroom hinge.

Claims Analysis

Total Claims: 5
Claim 1
Independent Claim 143 words
A trocar instrument for use in inserting a laparoscopic instrument into an abdominal cavity (24), the instrument comprising a tubular trocar sleeve (10) which is arranged to contain a trocar (12) with a sharp end of the trocar projecting from a first end (14) of the sleeve, whereby, in use, an incision (16) can be made in the abdominal wall (18) and the first end inserted through the incision into the abdominal cavity whereafter the trocar may then be withdrawn through the sleeve and replaced by the laparoscopic instrument; a radially expandable portion (20) at the first end (14) of the sleeve; and means (42,58) capable of expanding the portion when the first end has been inserted through the incision whereby the expanded portion abuts the inner abdominal wall (22) about the incision to resist withdrawal of the sleeve from the abdominal cavity.
Claim 2
Dependent Claim 67 words
An instrument according to claim 1, wherein the trocar sleeve includes a first sleeve portion (30) and a second sleeve portion (28), the sleeve portions being concentric relative to one another and the first sleeve portion having an expandable mushroom hinge (20) thereon, and the expanding means (42,58) causing the first and second sleeve portions (30,28) to move relative to one another to expand the mushroom hinge.
Claim 3
Dependent Claim 15 words
An instrument according to claim 1, wherein the expandable portion is an inflatable member (92).
Claim 4
Dependent Claim 17 words
An instrument according to claim 3, wherein the expanding means comprising a passage extending along the sleeve.
Claim 5
Dependent Claim 187 words
A trocar instrument for use in inserting a laparoscopic instrument into an abdominal cavity (24), the instrument comprising a tubular trocar sleeve (10) which is arranged to contain a trocar (12) with a sharp end of the trocar projecting from a first end (14) of the sleeve, whereby, in use, an incision (16) can be made in the abdominal wall (18) and the first end inserted through the incision into the abdominal cavity whereafter the trocar may then be withdrawn through the sleeve and replaced by the laparoscopic instrument; and a split seal (52) mounted within the sleeve, the split seal being formed of resilient material having a convex curvature facing the first end of the sleeve, a slit (76) being formed in the split seal to define first and second seal lips (78,80) facing each other, whereby, in use, gas pressure in the abdominal cavity urges the first and second seal lips into sealing engagement and insertion of the laparoscopic instrument into the passage is arranged to deflect the first and second seal lips apart to permit the instrument to be inserted into the abdominal cavity.
