Abstract

Abstract A novel method for replacing a previously implanted knee joint prosthesis in a patient is disclosed in which the previously implanted femoral component is replaced with a revision femoral component having thicker distal and posterior bearing portions so as to restore the natural anatomic transverse joint plane and collateral ligamentous balance in extension and flexion of the prosthetic joint. Instrumentation for use in this method includes a novel revision cutting jig which is used in the preparation of the distal end of the femur for the implantation of the revision femoral component.

Claim 1
Independent Claim 341 words
A method for surgically replacing in a patient a previously implanted knee joint prosthesis comprising a first femoral component and a first tibial component with a revision knee joint prosthesis comprising a second femoral component and a second tibial component while retaining the lateral and medial collateral ligaments intact and functional, with each of said first and second femoral components and first and second tibial components comprising a spaced pair of bearing members adapted to fit when implanted against the surgically prepared distal femur and proximal tibia, respectively, said first femoral component bearing members being adapted for mutual articulation with said first tibial component bearing members and said second femoral component bearing members being adapted for mutual non-linked articulation with said second tibial component bearing members, and the bearing members of said first and second femoral components including a distal portion adapted to contact the associated tibial component bearing member when the prosthetic knee joint is in extension and a posterior portion adapted to contact the associated tibial component bearing member when the prosthetic knee joint is in flexion, comprising the steps of (a) selecting a second tibial component; (b) selecting a second femoral component having bearing members with thicker distal and posterior portions than the bearing members of said first femoral component; (c) removing said first femoral component and said first tibial component from the body of the patient; (d) cutting the distal end of the patient's femur and the proximal end of the patient's tibia to prepare said femur and tibia to fit closely with said second femoral component and said second tibial component upon the implantation thereof; and (e) implanting said second femoral component and said second tibial component at the distal end of the patient's femur and the proximal end of the patient's tibia, respectively, with the dimensions of the bearing members of said second femoral component and said second tibial component being selected so that the natural anatomic transverse joint plane and collateral ligamentous balance are restored in extension and flexion of the prosthetic knee joint.
Claim 2

Claim 3
Dependent Claim 19 words
A method of claim 1 wherein said second tibial component has thicker bearing portions than said first tibial component.
Claim 4
Dependent Claim 28 words
A method of claim 1 wherein the bearing members of said second femoral component include additionally an anterior portion adapted to contact a prosthetic patellar component in use.
Claim 5
Dependent Claim 106 words
A method of claim 3 wherein the cutting of the distal end of the patient's femur in said step (d) comprises making a first horizontal planar cut across the distal end of the femur and then making two additional planar cuts at obtuse angles to said first planar cut and perpendicular to anterior-posterior planes, with one of said additional cuts being in the anterior region of the distal femur and the other of said additional cuts being in the posterior region of the distal femur, and wherein said three planar cuts are adapted to fit closely with three corresponding planar surfaces of said second femoral component.
Claim 6
Dependent Claim 30 words
A method of claim 4 wherein said additional planar cut in the posterior region of the distal femur is at an angle of about 109.5° to said first planar cut.
Claim 7
Dependent Claim 215 words
A kit for use in the surgical replacement in a patient of a previously implanted knee joint prosthesis comprising an implanted femoral component and an implanted tibial component, said kit comprising a plurality of revision tibial components comprising a spaced pair of bearing members adapted to fit when implanted against the surgically prepared proximal tibia and a plurality of revision femoral components comprising a spaced pair of bearing members adapted to fit when implanted against the surgically prepared distal femur, with the bearing members of each of said revision femoral components being adapted for mutual non-linked articulation with the bearing members of each of said revision tibial components, and the bearing members of each of said revision femoral components including a distal portion adapted to contact an associated revision tibial component bearing member when the prosthetic knee joint is in extension and a posterior portion adapted to contact an associated revision tibial component bearing member when the prosthetic knee joint is in flexion, with the thickness of the bearing members of each of said revision tibial components being different and with the thickness of the posterior bearing portions of each of said revision femoral components being the same and the thickness of the distal bearing portions of each of said revision femoral components being different.
