Abstract

Abstract An apparatus for the perfusion of cardioplegic solutions and related applications during heart surgery, suitable of handling both crystalloid and sanguineous solutions. The sanguineous type is prepared with a peristaltic pump (11) that features two pairs of rollers. The cardioplegic solution is fed to an heat exchanger (1) where the temperature is regulated to values between 4°C and 37°C by circulating a heat transfer fluid at a controlled temperature. The solution is sucked by a second peristaltic pump (14), passed through a filter (15a) and made available to the surgeon for the perfusion to the patient. Filter means (15), pressure and flow rate control means (19, 23) for the solution are provided. The exchanger (1) is disposable, as also are the various tubes (3, 5) and fittings used in preparing and perfusing the sanguineous solution. With the entire apparatus trolley-mounted and transportable, the perfusionist can prepare and distribute either type of solution without utilizing any auxiliary equipment or accessories.

Apparatus for the perfusion of cardioplegic solutions and similar applications during heart surgery, characterized in that it comprises: - heat exchange means in receipt of a heat transfer fluid and of said cardioplegic solutions, comprising an exchanger (1) affording inlet, ,outlet, vent and recycle ports (2b, 3b, 4a, 18, 4b) for said solutions, and a heat exchange element (7) in which said heat transfer fluid flows; - a first inlet circuit (2) through which a first cardioplegic solution is fed from a relative supply into the exchanger (1); - a second inlet circuit (3) through which a second, blended cardioplegic solution is fed into the exchanger (1), comprising means for blending the constituents of said solution; - a recycle and perfusion circuit (4, 5) connected to said heat exchange means by way of said outlet and recycle ports (4a, 4b), from which the cardioplegic solutions are returned to the exchanger and perfused to the patient; - means for circulating (6) said heat transfer fluid fed to said exchanger (1) and for controlling the temperature and flow rate thereof.
Claim 2
Dependent Claim 65 words
Apparatus according to claim 1, wherein said blending means of the second inlet circuit (3) comprise a first peristaltic pump (11) incorporating two pairs of rollers (43a, 43b) for flattening two tubes (9, 10) of dissimilar section, coming from a supply of crystalloid solution and from a blood oxygenator respectively, said tubes converging downstream of the pump into an inlet (3a) of said exchanger (1).
Claim 3
Dependent Claim 90 words
Apparatus according to claim 2, wherein said peristaltic pump (11) comprises a shaft (37) set in rotation by drive means (21), a fixed housing (39) affording an internal circular wall concentric with said shaft, a support (41) rigidly associated with said shaft and carrying two pairs of rollers (43a, 43b) occupying two distinct planes for flattening against said wall two flexible tubes (9, 10) arranged circumferentially along said wall, tube guide (40) means radially extending from said shaft (37) being provided for keeping said tubes spaced apart at unvarying distance.
Claim 4
Dependent Claim 78 words
Apparatus according to claim 3, wherein said support (41) comprises four pivots (42a, 42b) projecting from one face of said support, positioned at the same distance from the axis of rotation of said shaft (37), spaced apart at 90° from one another, at the ends of which said rollers (43a, 43b) are pivotally connected, each of said pivots being equal in lenght to the pivot diametrically opposite and dissimilar in lenght to the pivot adjacent on either side.
Claim 5
Dependent Claim 34 words
Apparatus according to claim 4, wherein said support (41) carrying said rollers (43a, 43b) comprises four identical arms arranged crosswise, at the end of each of them the pivots (42a, 42b) being rigidly associated.
Claim 6
Dependent Claim 59 words
Apparatus according to the claims 3, 4 and 5 wherein said tube guide means (40) comprises three pairs of radially extending crossed arms (40a, 40b, 40c) occupying planes disposed mutually parallel and equidistant, normal to the axis of the shaft (37), of which the ends are located a short distance from the internal circular wall of the housing (39).
Claim 7
Dependent Claim 45 words
Apparatus according to the preceeding claims, wherein circulation means (6) comprises a thermostatic vessel (28) filled with said fluid, and cooling means (25), heating means (26) and thermal sensing means (27) immersed therein for the regulation and the control of the temperature of said fluid.
Claim 8
Dependent Claim 32 words
Apparatus according to claim 7, wherein said cooling means (25) and heating means (26) immersed in the heat transfer fluid comprises respectively a refrigeration coil (25a) and an electrical resistance (26 ).
Claim 9
Dependent Claim 21 words
Apparatus according to claim 7, wherein said cooling means and heating means immersed in the transfer fluid comprise thermoelectric Peltier elements.
