Abstract

Abstract Method and apparatus are disclosed for thermally destroying a layer of an organ such as the mucosal layer of the gall bladder. The apparatus includes a catheter (100) having an elongated member (103) having a plurality of lumens therein. At the distal end of the elongated member is an electrode (104) for emitting radiofrequency current to the mucosal layer (101). Also at the distal end is a capacitive balloon electrode (106) surrounding the current-emitting electrode for containing an electrolyte solution (107) and for distributing the radiofrequency current to the mucosal layer (101). The balloon electrode is expanded with the electrolyte solution to conform and make contact with the mucosal layer. The electrolyte solution has a resistivity significantly less than the resistivity of the gall bladder wall, as well as the gall bladder bile, to cause a concentrated power deposition in the mucosal layer. The distal end of the catheter is endoscopically inserted into the body (114) of the gall bladder (102) by a retrograde route through the duodenum (117), common bile duct (118) and cystic duct (119). While the balloon electrode is being expanded, the bile present in the gall bladder is drained through one of the lumens in the elongated member. The apparatus also includes a radiofrequency generator (112) for supplying radiofrequency current to the current-emitting electrode. The current-emitting electrode is energized for a period of time to cause the mucosal layer to be heated for a predetermined period of time to thermally coagulate the mucosal layer of the gall bladder and cystic duct. A dispersive electrode (120) is positioned on the skin of the patient's body to facilitate a complete circuit back to the generator without causing trauma to the patient.

Claims Analysis

Total Claims: 9
Claim 1
Independent Claim 111 words
Apparatus for thermally destroying or coagulating living tissue forming an inner wall in part of the body of a patient, said apparatus comprising an elongated member (100) with an RF emitter arrangement (104) at a distal end (105) thereof, and an extendable arrangement (106) which when extended would conform to the normal contour of the said inner wall and which when extended would serve to position or locate the RF emitter arrangement within the said inner wall, characterized in that means (106) are provided for enclosing and maintaining liquid (bile or 107) within the region between the emitter arrangement and the said inner wall whilst the latter is being thermally destroyed.
Claim 2
Dependent Claim 111 words
Claim 3
Dependent Claim 45 words
Apparatus according to claim 1, in which the apparatus is designed to destroy the inner wall of a gall bladder, characterized in that the enclosing and maintaining means serves to seal the liquid within the gall bladder whilst the said inner layer is being destroyed.
Claim 4
Dependent Claim 59 words
Apparatus according to claim 1 or 2, characterized in that the extendable arrangement and the enclosing and maintaining means comprise a balloon, and in that means (201 and 202) are provided for supplying liquid to expand the balloon and for withdrawing at least some of the bile from the gall bladder prior to the operation of the emitter arrangement.
Claim 5
Dependent Claim 66 words
Apparatus according to claim 3, characterized in that a lumen (202) is provided for withdrawing the bile from the region between the exterior of the balloon and the said inner wall, said lumen (202) communicating directly with the exterior of the balloon, and in that a second lumen (201) communicating with the interior of the balloon is provided for supplying said liquid to expand the balloon.
Claim 6
